Surname If you ever been known by another surname you must declare any or all of them (unless you were adopted before the age of 10 and you name changed before then). If you use the title “Ms” but have never changed your surname you must still put your current (and only) surname in this box and the current year in box C21. Previous Forenames If you’ve used previous forenames (especially if you might have opened back accounts or applied for official documents) please put all these here. If you need additional boxes, download a Continuation sheet.

 

Where you were born

Please put your place of birth as it appears on your birth certificate.

 

Past Addresses

If you have not lived at your current address for more than 5 years, please put your last address here. If these two addresses do not cover 5 years then download a Continuation sheet and put all previous addresses until at least the last 5 years addresses are covered.
